Investigators sought a peptide that would preserve the cognitive effects associated with ACTH while eliminating the hormonal stimulation of the adrenal cortex. The compound entered clinical use in Russia during the following decade.
Russian regulatory authorities approved the peptide for nasal administration, and it remains listed in the national pharmacopoeia under several trade names. Documented indications include acute ischemic stroke, transient ischemic attacks, traumatic brain injury, and certain ophthalmological and neurological conditions. Physicians also prescribe it for cognitive complaints in older patients, although the evidence base for that use is thinner. Systematic reviews have generally described the evidence base as limited in size and variable in methodological quality. Randomised controlled data suitable for international regulatory assessment are scarce. As a result, major treatment guidelines outside Russia do not include the peptide, and interest in it remains largely research-driven rather than routine clinical.

There is an ongoing opioid epidemic (also known as the opioid crisis) in the United States, originating out of both medical prescriptions and illegal sources. It has been described by doctors and researchers as "one of the most devastating public health catastrophes of our time". The opioid epidemic unfolded in three waves. The first wave of the epidemic in the United States began in the late 1990s, according to the Centers for Disease Control and Prevention (CDC), when opioids were increasingly prescribed for pain management, resulting in a rise in overall opioid use throughout subsequent years. The second wave was from an expansion in the heroin market to supply already addicted people. The third wave, starting in 2013, was marked by a steep tenfold increase in the synthetic opioid-involved death rate as synthetic opioids flooded the US market. In the United States, there were approximately 109,600 drug-overdose-related deaths in the 12-month period ending January 31, 2023, at a rate of 300 deaths per day. From 1999 to 2020, nearly 841,000 people died from drug overdoses, with prescription and illicit opioids responsible for 500,000 of those deaths. Drug overdose deaths totaled 70,237 in 2017, with 47,600 involving an opioid. This surpassed deaths from car crashes (37,133) by 28%. A December 2017 report estimated that 130 people die every day in the United States due to opioid-related drug overdose. The great majority of Americans surveyed in 2015 who used prescription opioids did not believe that they were misusing them.
The National Institutes of Health (NIH) is the primary agency of the United States federal government responsible for biomedical and public health research. It was founded in 1887 and is part of the United States Department of Health and Human Services (HHS). == Dosage and administration == The drug and its generic counterparts are supplied in packages of 12, and available in two strengths: Each B&O Supprettes suppository #15 A contains 16.2 mg (1/4 grain) of belladonna and 30 mg (1/2 grain) of opium. Each B&O Supprettes suppository #16 A contains 16.2 mg of belladonna and 60 mg (1 grain) of opium. The usual dose is one suppository rectally once or twice daily PRN - (as needed), not to exceed four Supprettes in a 24-hour period. In the United States, B&O Supprettes is a Schedule II drug under the Controlled Substances Act of 1970; a written prescription is mandatory, and no refills are permitted. Refrigerated storage is preferable, but not required. Most pharmacies consider B&O Supprettes to be a "special order" item, and as such are not normally kept in inventory. Compounding pharmacies have the capability of producing a generic form of the medication, and can modify the dosage(s) of the active ingredients (for pediatric or elderly patients, and those with chronic kidney disease) or the carrier (usually substituting cocoa butter) to best meet the needs of the patient at the request of the prescriber.

=== pH buffering === The pH of the mobile phase can change the retention and selectivity of analytes. For samples containing solutes with ionized functional groups, such as amines, carboxyls, phosphates, phosphonates, sulfates, and sulfonates, the ionization of these groups can be controlled by controlling the pH of the mobile phase. The pH of the mobile phase can be controlled by mobile phase buffers. In general, molecules with acidic function groups are increasingly ionized under more alkaline environments, and thus its retention time would decrease. Conversely, under more acidic environments, its retention time would increase. Conversely for molecules with alkaline function groups. For example, carboxylic groups in solutes become increasingly negatively charged as the pH of the mobile phase rises above their pKa, hence the whole molecule becomes more polar and less retained on the a-polar stationary phase. In this case, raising the pH of the phase mobile above 4–5 = pH (which is the typical pKa range for carboxylic groups) increases their ionization, hence decreases their retention. Similarly, molecules with amine groups usually have pKa around 8. However, since silica gel is not stable in alkaline environments, the mobile phase typically does not have pH above 8. Therefore, control over the retention of amines is limited when the stationary phase is based on silica gel. The choice of buffer type is an important factor in RP-LC method development, as it can affect the retention, selectivity, and resolution of the analytes of interest.
Compounds with a high ratio of androgenic to an anabolic effects are the drug of choice in androgen-replacement therapy (e.g., treating hypogonadism in males), whereas compounds with a reduced androgenic:anabolic ratio are preferred for anemia and osteoporosis, and to reverse protein loss following trauma, surgery, or prolonged immobilization. The VP weight is an indicator of the androgenic effect, while the LA weight is an indicator of the anabolic effect. Two or more batches of rats are castrated and given no treatment and respectively some AAS of interest. The LA/VP ratio for an AAS is calculated as the ratio of LA/VP weight gains produced by the treatment with that compound using castrated but untreated rats as baseline: (LAc,t–LAc)/(VPc,t–VPc). The LA/VP weight gain ratio from rat experiments is not unitary for testosterone (typically 0.3–0.4), but it is normalized for presentation purposes, and used as basis of comparison for other AAS, which have their androgenic:anabolic ratios scaled accordingly (as shown in the table above).
