=== The Fish Farmers clan === The Fish Farmers clan (Dutch: De Visboeren) are led by the owners of the well-known fishmonger De Antwerpse Visser in the districts of Borgerhout and Kiel in Antwerp. The organisation is led by the brothers Mohamed A., Rachid A. and Hassan A., who quickly established themselves as main contributors within the international cocaine trade. They established the organisation in the early 2010s and took control over the cocaine trade throughout Flanders. Dozens of containers from Colombian cartels were trafficked through the organisation of the Fish Farmers clan. They very quickly competed against other organisations within the Moroccan mafia. In 2018, the Kali-team (an anti-Moroccan mafia police unit from Antwerp) discovered that the Fish Farmers were amongst the main owners of a luxury suite in the Hilton hotel near the Tangier-Ville railway station in Morocco. In March 2020, the three brothers and their father Mohamed (73) were sentenced up to eight years in prison.
== Functional protein microarrays == Functional protein microarrays (also known as target protein arrays) are constructed by immobilising large numbers of purified proteins and are used to identify protein–protein, protein–DNA, protein–RNA, protein–phospholipid, and protein–small-molecule interactions, to assay enzymatic activity and to detect antibodies and demonstrate their specificity. They differ from analytical arrays in that functional protein arrays are composed of arrays containing full-length functional proteins or protein domains. These protein chips are used to study the biochemical activities of the entire proteome in a single experiment. The key element in any functional protein microarray-based assay is the arrayed proteins must retain their native structure, such that meaningful functional interactions can take place on the array surface. The advantages of controlling the precise mode of surface attachment through use of an appropriate affinity tag are that the immobilised proteins will have a homogeneous orientation resulting in a higher specific activity and higher signal-to-noise ratio in assays, with less interference from non-specific interactions.

It is not as widely used as buprenorphine or methadone for OUD due to low rates of patient acceptance, non-adherence due to daily dosing, and difficulty achieving abstinence from opioids before beginning treatment. Dosing naltrexone after recent opioid use can lead to precipitated withdrawal. Conversely, naltrexone antagonism at the opioid receptor can be overcome with higher doses of opioids. Naltrexone monthly IM injections received FDA approval in 2010 for the treatment of opioid dependence in abstinent opioid users.
Choose the right targets Continuous research into the pathophysiology of stroke has led to improved ability to select drugs targets. Acute ischemic strokes start when there is reduced blood flow, often caused by an occlusion, to part of the brain. Even if an occlusion causes a complete blockage of a major artery, there is typically still some blood flow downstream of the blockage through collateral blood vessels. With reduced blood flow, there is reduced oxygen supply, and to compensate the tissue goes through anaerobic metabolism which is much less efficient. If anaerobic metabolism does not provide enough energy, there is energy failure, followed by ion imbalances. Afterwards, the pathophysiology gets complicated and there are thought to be at least eight pathways of tissue damage. By targeting processes near the top of the top of the chain of events, problems further down the chain of events can be avoided. For example, the drug tPA and mechanical thrombectomy devices all target the occlusion which is at the top of the chain of events, and have achieved FDA approval. The next step in the chain of events is hypoxia, and some oxygen delivery drugs have shown strong effects in animal studies, as shown in the table below. If processes further down the chain of events get targeted, there may be many simultaneous problems and the effect of a single therapy may be less, so there may be benefit to using multiple drugs in combination to treat multiple pathways.

=== Pharmacological === The medications available differ between substances. For opioid use disorder, opioid agonist maintenance treatment with methadone or buprenorphine is common. It retains people in treatment and suppresses illicit opioid use more effectively than placebo. A systematic review and meta-analysis of 15 trials and 36 cohort studies found that time spent receiving opioid agonist treatment was associated with lower all-cause and overdose mortality, with the risk of death around six times higher in the four weeks after treatment stopped. A Cochrane review also found that opioid substitution therapy reduces the risk of acquiring hepatitis C among people who inject drugs, with a larger reduction when combined with high-coverage needle and syringe programmes. For alcohol use disorder, oral naltrexone and acamprosate are recommended as first-line medications alongside psychosocial support, whereas trial evidence that disulfiram reduces drinking is weaker. For tobacco (nicotine) dependence, varenicline, cytisine and nicotine e-cigarettes are among the most effective aids for quitting long-term, followed closely by using two forms of nicotine replacement therapy at once (for example a patch together with gum or a lozenge). Bupropion is also used. No medication has been approved for cocaine or amphetamine (stimulant) use disorder, for which psychosocial treatment remains the principal care.

== Regulation == The primary control of cortisol is the pituitary gland peptide, adrenocorticotropic hormone (ACTH). Once released from the pituitary, ACTH travels in the general circulation to the adrenal gland where it binds to melanocortin 2 receptors which stimulate cortisol production. ACTH is in turn controlled by the hypothalamic peptide corticotropin-releasing hormone (CRH), which is under nervous control. CRH acts synergistically with arginine vasopressin, angiotensin II, and epinephrine. (In swine, which do not produce arginine vasopressin, lysine vasopressin acts synergistically with CRH.) When activated macrophages start to secrete IL-1, which synergistically with CRH increases ACTH, T-cells also secrete glucosteroid response modifying factor (GRMF), as well as IL-1; both increase the amount of cortisol required to inhibit almost all the immune cells. Immune cells then assume their own regulation, but at a higher cortisol setpoint. The increase in cortisol in diarrheic calves is minimal over healthy calves, however, and falls over time. The cells do not lose all their fight-or-flight override because of interleukin-1's synergism with CRH. Cortisol even has a negative feedback effect on interleukin-1—especially useful to treat diseases that force the hypothalamus to secrete too much CRH, such as those caused by endotoxic bacteria. The suppressor immune cells are not affected by GRMF, so the immune cells' effective setpoint may be even higher than the setpoint for physiological processes.
