Semax is a synthetic heptapeptide with the sequence Met-Glu-His-Phe-Pro-Gly-Pro. Its chain combines the first seven residues of corticotropin with a C-terminal proline-glycine-proline extension, a modification intended to slow enzymatic breakdown. The free peptide has a molecular mass near 813.9 daltons. It belongs to the class of ACTH-derived fragments studied for central nervous system activity rather than for adrenal steroid stimulation. This structural relationship to a natural hormone fragment is the usual starting point for describing the compound in the literature.
Development took place during the 1980s at institutes of the Russian Academy of Sciences, where peptide fragments of ACTH were screened for cognitive and neuroprotective effects. The compound received regulatory approval in Russia as a nasal preparation, marketed for conditions such as ischemic stroke, transient ischemic attacks, and optic nerve disorders. Registration in other countries has not followed. Most published clinical reports originate from a small number of Russian research groups, and independent replication outside that setting remains limited. The regulatory status therefore differs sharply between Russia and the rest of the world.
Development of the compound took place in the Soviet Union during the 1980s, largely through peptide research groups affiliated with the Shemyakin-Ovchinnikov Institute and Moscow State University. Investigators screened fragments of adrenocorticotropic hormone for activity on the central nervous system while attempting to separate cognitive effects from hormonal ones. The shortened sequence was selected because it retained behavioral activity in animal models without stimulating corticosteroid release. Early publications described the molecule as an ACTH(4-10) analog.
Published research on this peptide originates mainly from Russian laboratories, and the wider international literature is comparatively thin. Studies have used rodent models of stroke, hypoxia, and memory tasks, with a smaller number of human trials reported. Reported outcomes include changes in attention and memory measures, along with effects on neurotrophic factor expression in some experiments. Small sample sizes, inconsistent dosing protocols, and limited independent replication are widely noted limitations, so the strength of the clinical evidence remains an open question.

However, by 24 hours after the dose, unchanged EMP could no longer be detected in the circulation. The clearance of EMP from blood plasma is 4.85 ± 0.684 L/h. The volumes of distribution of EMP with intravenous injection were small; under a two-compartment model, the volume of distribution for the central compartment was 0.043 L/kg and for the peripheral compartment was 0.11 L/kg. The plasma protein binding of EMP is high. Estramustine is accumulated in tumor tissue, for instance prostate cancer and glioma tissue, with estramustine levels much higher in these tissues than in plasma (e.g., 6.3- and 15.9-fold, respectively). Conversely, levels of estromustine in tumor versus plasma are similar (1.0- and 0.5-fold, respectively). Estramustine and estromustine appear to accumulate in adipose tissue. The elimination half-life of estromustine with oral EMP was 13.6 hours on average, with a range of 8.8 to 22.7 hours. Conversely, the elimination half-life of estromustine with intravenous injection was 10.3 hours, with a range of 7.36 to 12.3 hours. For comparison, the corresponding elimination half-lives of estrone were 16.5 and 14.7 hours for oral and intravenous administration, respectively. Estramustine and estromustine are mainly excreted in bile and hence in feces. They are not believed to be excreted in urine.
Formation of additional capillaries and larger blood vessels (angiogenesis) is a major mechanism by which a cancer may help to enhance its own growth. Disorders of retinal capillaries contribute to the pathogenesis of age-related macular degeneration. Reduced capillary density (capillary rarefaction) occurs in association with cardiovascular risk factors and in patients with coronary heart disease.

== Cambridge Dictionary == The Cambridge Dictionary Word of the Year, by Cambridge University Press & Assessment, has been published every year since 2015. The word is chosen based on "user data, zeitgeist, and language." In 2024, Cambridge picked "manifest" as its Word of the Year. Traditionally, the word has been used as an adjective meaning "obvious", or as a verb meaning "to show something clearly through signs or actions". The word was chosen owing to its use by celebrities, particularly on social media, as a verb meaning "to imagine achieving something you want, in the belief doing so will make it more likely to happen".
=== Signs and symptoms === Riboflavin deficiency (also called ariboflavinosis) results in stomatitis, symptoms of which include chapped and fissured lips, inflammation of the corners of the mouth (angular stomatitis), sore throat, painful red tongue, and hair loss. The eyes can become itchy, watery, bloodshot, and sensitive to light. Riboflavin deficiency is associated with anemia. Prolonged riboflavin insufficiency may cause degeneration of the liver and nervous system. Riboflavin deficiency may increase the risk of preeclampsia in pregnant women. Deficiency of riboflavin during pregnancy can result in fetal birth defects, including heart and limb deformities.
=== Reaction === Limulus clotting enzyme uses 2H2O to cleave the Arg18-Gly19 and Arg46-Thr47 linkages in coagulogen, forming coagulin + two peptide products. This process activates coagulin, allowing it to gel and form clots around bacterial invaders. The reaction is hydrolytic and catalyzed by the His-Asp-Ser triad in the active site. The hydroxyl group in serine acts as a nucleophile, attacking the carbonyl carbon in the peptide bond. Histidine’s nitrogen atom accepts the hydrogen from serine’s hydroxyl group to make serine a stronger nucleophile. Aspartic acid forms hydrogen bonds with histidine to make this nitrogen atom more electronegative and therefore more likely to accept the hydrogen. The nucleophilic attack results in the peptide bond breaking, allowing one peptide fragment to be released. A water molecule is then deprotonated by the histidine. The resulting hydroxyl acts as a nucleophile to break the bond between the serine and the carbonyl, releasing the rest of the protein. The process is then repeated at the other peptide bond.
== Age-related prevalence of the CKM syndrome == A study of 231,590,853 individuals living in the USA found older aged patients trended to have higher stages of the CKM syndrome. In the 45 to 64 years old group, 5.4% were stage 0, 21.0% were stage 1, 63.8% were stage 2, 1.2 % were stage 3, and 8.6% were stage 4. In the 65 years and older age group, 1.8% were stage 0, 8.4% were stag 1, 39.0% were stage 2, 24.6% were stage 3, and 26.2% were stage 4. The 20 to 44, 45 to 64, and 65 or greater years old groups consisted respectively of 104,113,793, 82,640,261, and 44,836,799 individuals.
== Chemistry == Vitamin B12 is the "generic descriptor" name for any vitamers of vitamin B12. Animals, including humans, can convert cyanocobalamin to any one of the active vitamin B12 compounds. Cyanocobalamin is one of the most widely manufactured vitamers in the vitamin B12 family (the family of chemicals that function as B12 when put into the body), because cyanocobalamin is the most air-stable of the B12 forms, and is the easiest to purify after it is produced by bacterial fermentation. It can be obtained as dark red crystals or as an amorphous red powder. Cyanocobalamin is hygroscopic in the anhydrous form, and sparingly soluble in water (1:80). It is stable to autoclaving for short periods at 121 °C (250 °F). The vitamin B12 coenzymes are unstable in light. Vitamin B12 additionally becomes inactive when exposed to intense heat or electromagnetic radiation.

Semax is a synthetic seven-amino-acid peptide derived from a fragment of corticotropin. It is used in Russia as a nasal preparation, while elsewhere it is studied as a research compound. It is not approved as a medicine in most countries.
Its sequence corresponds to the 4-10 region of corticotropin extended by three residues. This fragment lacks the region responsible for adrenal steroid release, so it is explored for neural rather than hormonal effects. The relationship explains the compound's classification among ACTH-derived peptides.
It holds approval in Russia for several neurological indications as a nasal spray or drop formulation. Regulatory agencies in the United States and the European Union have not approved it. Products sold elsewhere are generally labeled for research use only.
It is a synthetic peptide of seven amino acids. Its backbone corresponds to a fragment of adrenocorticotropic hormone extended at the carboxyl end. The material is distributed as a lyophilized solid for laboratory use.
